How to File Maternity on SSS

1st - Filing of Maternity Notification MAT1:

Via SSS Website: (If you have an online account)
1. Log-in
2. Click Transaction --> Maternity Notification
3. Enter the Required Information
4. Then Submit.
          You will receive an email confirmation telling that your notification was a success or not.

Via SSS Office:
1. Ask for a Maternity Notification form or MAT1 
2. Then Submit to the responding counter.
3. Bring anything that will prove your pregnancy (ex. ultrasound)

2nd - Filing of Maternity Notification MAT2

1. Go to SSS office or branch near you
2. Ask for MAT 2 form then fill-up
3. Then proceed to the counter and submit all the necessary requirements.

REQUIREMENTS (based on SSS MAT2 back form)
1. Maternity notification duly stamped received by the SSS prior to the date of childbirth/miscarriage
2. The member must have paid at least three monthly contributions within the 12 month period immediately preceding the
semester of childbirth or miscarriage.
3. a. For Normal Delivery
• Certified true copy or authenticated copy of Birth Certificate duly registered with the Local Civil Registrar.
b. For Cesarean Delivery
• Certified true copy or authenticated copy of Birth Certificate duly registered with the Local Civil Registrar.
• Operating Room Record or Surgical Memorandum duly certified by the hospital where the member is confined.
c. For Stillbirth
• Fetal Death Certificate duly registered with the Local Civil Registrar.
d. For Miscarriage or Abortion
• Pregnancy test before and after miscarriage/abortion.
• Medical Certificate/Obstetrical history indicating the number of miscarriages duly certified by the attending physician
with his license number, printed name and signature; or
• D & C Report duly certified by the authorized hospital representative where the member was confined.
4. a. For Separated Member
• Certification from last employer with the effective date of separation from employment.
b. For Voluntary Member
• a copy of approved SS Form E-5
c. For Self-employed Member
• a copy of approved SS Form RS-1

IMPORTANT: (based on my own experience)
     If you gave birth abroad, you have to bring and photocopy all the documents that will prove that you really gave birth outside of our country (Philippines). Very important is to bring the original/photocopy of birth registration from the embassy of the country you are in.
